Transforming Prayer Request Form

Transforming Prayer is a gentle, yet powerful tool for inner healing. It is not counseling. The goal of Transforming Prayer is to connect with God to uncover and address root issues that are hinderances to your growth and relationship to God and others.

What To Expect
There will be 2-3 trained prayer ministers in the room who are praying with you and guiding you through the process of connecting with God as the Holy Spirit leads. You will be invited to ask Father God, Jesus, and the Holy Spirit questions about wounds that need healing, lies you have been believing, people you may need to forgive, and other issues that are hindering your freedom and healing. You will come away with fresh revelation of God’s love for you and His truths for your life.

Liability Release Form

The purpose of the Riverstone Transforming Prayer Ministry (Prayer Ministry) is for spiritual growth and
healing. It is not a psychological counseling service, nor is it intended to be. Prayer Ministry team members are
not licensed psychologists or psychiatrists; however, they are trained in using the Word of God and prayer
through the power of the Holy Spirit. The results of this approach depend on the willingness of the individual
to make wise choices consistent with the teachings of Jesus Christ.

All personal information gathered in the course of a Transforming Prayer Ministry session is confidential and
any written or oral information obtained from or created on behalf of a participant are so maintained. My
information, as deemed necessary by the Transforming Prayer Ministry, may be shared with other members of
the Transforming Prayer Ministry to further my healing process and I consent to such sharing. Transforming
Prayer Ministers may consult with other Transforming Prayer Ministry team members without disclosing
participant’s names. Training others in the mastery of prayer ministry skills is an ongoing aspect of prayer
ministry. Qualified individuals may be assigned to be present during ministry sessions. The adherence to a
strict policy of confidentially includes all individuals working with prayer ministers.

Riverstone Church will abide by all laws regarding the reporting of threats to self, threats to others, abuse of a
child, abuse of the elderly and/or other behavior deemed by Riverstone Church and the Transforming Prayer
Ministry to be harmful to yourself or others. Regardless of local laws, and out of care and respect for God’s
creation, Riverstone will report any of the above to the proper authorities.

No Transforming Prayer session shall be recorded unless Riverstone and the participant(s) agree and the
participant consents, in writing.

Riverstone Church may agree to provide Transforming Ministry services to a minor with the approval and the
written consent of a parent or legal guardian.

I understand the above information concerning this Ministry. I have sought this Ministry of my own free will
and am under no obligation to accept or reject any of the advice or help that I might receive from the Ministry
Team members or from any individual acting on behalf of Riverstone Church. All personal information I reveal
is given voluntarily in order to facilitate the team members working with me. I further release and hold
harmless Riverstone Church, its employees and agents from any and all liability, loss or damage of any kind
that may arise or result from these ministry sessions. This consent and release remains in effect unless and
until revoked by the participant, in writing.
